An EOB should be submitted after each infusion treatment with CINQAIR^®. However, EOBs can be submitted at any time while you are enrolled and still eligible for the program.

  If you become ineligible for the CINQAIR^® Cost Support Program, you can still submit EOBs for treatments you received when you were eligible for the program.
